Traumatic diaphragmatic rupture (TDr) is a rare condition that can cause serious problems if unrecognized. We report a rare case of intrathoracic herniation of liver caused by a right sided TDr in a patient sustained crush injury that was operated on with provisional diagnosis of laceration of the liver. The liver was retracted from the chest through the diaphragmatic tear and replaced in the abdomen. The diaphragmatic defect repaired with non absorbable suture in two layers and the patient discharged uneventfully. Diagnosis of right sided TDr occasionally can be overlooked despite all imaging modalities.
