In this paper, a smart inhaler monitoring device is introduced for designing to improve medication adherence of inhalers among asthmatic patients. Asthma is a chronic and prevalent respiratory disease globally, and inhaled therapy is the main part of the treatment strategy. The correct administration of inhaler medication and adherence to treatment are, however, often poor, which affects asthma control in a negative way. A smart inhaler monitor device consisting of a small electronic module that could be mounted on the inhaler drug was developed in this work and its prototype was fabricated. The device
has an accelerometer sensor to detect shaking of the medicine in the correct manner and administration of the same at the correct angle, an ESP8266 Wi-Fi module to provide data communication to patient relatives and healthcare professionals, and an OLED and warning LEDs display to provide user feedback. The performance of the developed device was verified using various test cases under lab conditions. It is proven that the accelerometer sensor can measure correctly the proper shaking time and angle of the inhaler medicine. The device information was successfully transmitted to a cloud platform (ThingSpeak) using the Wi-Fi module and the patient’s relative/doctor can access it remotely. It was observed that the OLED screen and warning LEDs display medication instructions and device status to the user in an understandable way. The results of this study show that the smart inhaler tracking device has the potential to enhance inhaler medication adherence in asthma patients and follow-up of patients. The device is thought to be capable of assisting patients to improve medication use skills, increase adherence to treatment, and provide healthcare providers with better patient management. In future works, the device’s performance on patients in the clinical setup and patient satisfaction will be evaluated and its commercial potential will be decided.
