A case of high complex transphincteric perianal fistula with secondary high intersphincteric tract is presented. The patient was managed successfully with endoanal advancement flap (EAAF) surgical procedure and followed up to 6 months with complete healing of the fistula. EAAF is a good surgical option for management of this challenging fistula.
